The Switch Kit is a small group of simple forms to help streamline the process of moving your accounts from your previous bank to State Bank. If you need help completing your Switch Kit, please contact us, and we will be glad to help you through the process.

2. Save time by completing the Switch Kit forms on your computer while viewing it through Adobe Reader, or print the kit and complete by hand.
Switch Kit Time Saver: Once you fill in information on your computer such as your name and address that is needed on more than one form, the Switch Kit saves you time by automatically filling in that information within the other forms.
3. Once completed, print your Switch Kit and sign each form where necessary.
4. Bring your Switch Kit to the State Bank and a customer service representative will assist you in completing the process.

The following forms below are all included within the Switch Kit download. If you need to print out just one or a few copies of one specific form you can do so by downloading the form below.
Your new State Bank checking or savings account opens the door to a number of benefits. Complete the New Account Information form and ask to speak with a State Bank customer service representative. He or she will take care of setting up your account(s). We'll even order your new checks. Photo identification is required when you open new accounts.

To switch any direct deposits you may have set up with your employer or the company handling your retirement or pension payments, complete the Change Direct Deposit form. A State Bank customer service representative will take care of the rest. Be sure to include a voided check (most companies require it to verify your account number).

You'll need to contact the companies and financial institutions with which you have established automatic withdrawals. Complete the Change Automatic Withdrawal form, and a State Bank customer service representative can take care of the rest. Be sure to include a voided check (most companies require it to verify your account number).

Leave your old accounts open with enough money to cover outstanding checks and automatic withdrawals. Once the accounts are no longer active, send your former financial institution the Close Account Form and request the balance from the accounts. Finally, destroy old checks, ATM/debit cards and deposit slips.
